Access to Care: We will care for patients regardless of age, sex, race, gender, creed, religion, or disability. Walk-ins are welcome and same day appointments are available. Our hours are Monday through Thursday, 7:00 a.m. to 5:00 p.m. and Friday, 7:00 a.m. to 4:00 p.m.
After Hours: A physician is on call at all times and can be reached by calling our number and reaching the answering service. Please use this service for urgent medical problems that cannot wait until normal office hours.
Urgent Problems: Please call our office for any urgent problems during office hours and notify the receptionist so she can get the message to the appropriate person. Most urgent problems will require an office visit. Same day appointments are available. We welcome walk-ins.
Emergencies: Please call 911 for any medical emergencies.
New patient office Visit: Prior to any office visit, please arrive 15 minutes early to complete paperwork. Download the helpful forms from this web site and complete them prior to your office visit. Please bring your insurance card and picture ID to each office visit. Bring your medications, immunization records, and complete a medical records request to obtain your old records.
Referrals: Referrals to other physicians or diagnostic testing facilities will be made by this practice within 5 working days after receipt of the physician’s orders. Emergency referrals will be scheduled immediately. It is very important that our Referral Coordinator have current and accurate insurance information available in your chart.
Test Results: Should you have any laboratory work or other diagnostic testing done through this practice, you will be notified of the results as soon as they are available (usually within 7 working days from test date). All test results must first be reviewed by the physician. You will then receive a call or a mailed note from the physician’s medical assistant with the physician’s instructions for further consultation or testing. Please notify us if you do not receive your test or lab results after 2 weeks.
Prescriptions: All prescriptions and refill requests should be requested during normal office hours, preferably Monday through Friday in order to facilitate the request. Your refill request should be submitted to your pharmacy first and they will then call the office for authorization. Your refill requests will be handled by this practice within 48-hours after your pharmacy’s request is received. Prescriptions for narcotics and antibiotics will ALWAYS require an office visit.
Records Release: Records will be released to any physician upon your written request and authorization as a courtesy. However, for personal records release requests, we charge a fee of $10.00 for the first 10 pages, then $0.50 . per page for pages 11-50, and $0.33 per page for every additional page. The charge for making a CD of your medical records is $14.00.
Telephone Messages: Telephone messages will be returned by the provider or a medical assistant as soon as possible. Many times the provider will have the medical assistant answer your questions or instruct you to make an appointment to be seen. Should your message be urgent, please advise the receptionist. Messages to the Doctor’s medical assistant will be returned within 24 hours. Urgent telephone messages that require immediate help should be directed to the receptionist to and she will then relay the message to the doctor as soon as possible.
Financial Policy: Copays and self pays are expected at the time of service. We accept all major credit cards, personal checks, and cash. Any check returned from the bank will result in an additional $35 dollar charge that will appear on your account. If your insurance carrier has not responded to the claim within 90 days, we reserve the right to transfer the liability of the claim to the patient/guarantor. For any billing question, please call our billing representative. We realize that emergencies do arise that will effect timely payment of your account. If extreme cases do occur, please contact our patient billing representative. Please notify our office of changes in your name, address, phone number, or insurance information.
Privacy: Your privacy is important to us. We make every effort to comply with HIPAA federal regulations. Should you have any concerns or questions, please notify us. Please read a copy of our “Health Information Practices” from our receptionist.
Appointment Cancellation/Rescheduling: If you are unable to make your scheduled appointment for any reason, we require a 24 hour notification. This will allow us to fill your appointment slot with someone who may need to be seen urgently. Missed appointments will be charged a $50.00 fee.
Email:If you decide to use email to contact us, remember that this is not a secure form of communication and anything of a private or personal nature should be avoided. For simple refill requests or questions, this is appropriate. Email will be answered within 24-48 hours. Do not use email for urgent or emergent matters.
